Medically prescribed coordinated services which are appropr-late to the plan of care and will enhance the patient's return to independent activities of daily living.

Who will need Home Health Care? 

  • Patients who needs short term assistance after discharge from the hospital.

  • Chronically ill, terminally ill, or permanently disabled who wish to remain at home.

  • Older adults, who due to illness or disability, need assistance in their homes.

  • Family respite, to provide short term relief for family members caring for a patient or relative who is ill

Who pays for Home Health Care services? 

  • Insurance Coverage

    • We will verify upon receipt of any assignment of benefits.

    • We will bill your insurance carrier directly.

  • MEDICARE/MEDICAL

    • If you are eligible for home health care coverage through Medicare or Medical our office will bill them directly.

  • Direct Payment (private patients only)

    • When billed directly to you, HOME CARE statement clearly dentifies visits and hours spent by our home health care professionals. You pay only per visit. 

What are the benefits of Home Health Care? 

  • Care is provided in the privacy and comfort of your home.

  • All services are coordinated with your physician(s).

  • 24 hour access to a skilled nurse.

  • Home care may prevent hospitalization/re-hospitalization.

  • Short recovery time.

What types of conditions would indicate a need for Home Health Care?

  • New Surgical Procedure

  • New or changed medical diagnosis

  • New or changed medication

  • New or changed caregiver

  • New medical equipment

  • Difficulty in walking, transferring, or performing regular daily activities.
